Who registers the death?

Where the death happenedWho reports itTo whom
Government hospital with a registration unitThe hospital (medical officer in charge)Registered at the hospital itself
Private hospital / nursing homeThe hospitalRegistrar of the area where the hospital is
At home — villageHead of household or nearest relative present (or oldest person present)Local registrar — panchayat / health official
At home — town or cityHead of household or nearest relative presentMunicipal registrar (health officer of the nagar nigam / nagar palika)

Deaths are registered at the place where they occurred, not the family’s permanent address. ASHAs, ANMs and anganwadi workers can also notify the registrar.

Step by step

  1. Keep documents ready: medical certificate of cause of death (from the hospital/doctor, if available), ID of the deceased (Aadhaar or other), informant’s ID, and proof of the date and place of death.
  2. Report within 21 days to the registrar listed above. For hospital deaths, confirm with the hospital that it has been reported.
  3. Track and collect: after registration, the registrar issues the certificate — the law now requires it within seven days of registration, electronically or otherwise. Where online access is enabled, it can be downloaded via the CRS portal (dc.crsorgi.gov.in).
  4. Check every detail — name, father’s/husband’s name, date and place. Corrections are possible later under section 15 of the RBD Act, but take time.

Fees and copies

One copy is issued free to the informant (section 12 of the RBD Act). Extra copies can be obtained by anyone on paying the prescribed fee (section 17). Banks, insurers and pension offices often keep a copy, so take a few.
